Stop the video when your push away arm is fully extended and just about to drop into the swing. The ball is too far away from your body. This is why your body is leaning forward to early. The 3rd step in a 5 step approach is good to start leaning forward. What you need to do is a hinge swing.

Watch Barnes and Malott doing the hinge watch how much their elbows move forward in the push away.
Barnes-  http://www.youtube.com/watch?v=u_Ksq0kgKNo&feature=related
Malott-  http://www.youtube.com/watch?v=sXoom5Hj59Q&feature=PlayList&p=72855992A6EE73A8&index=43
With Malott his forward lean isn't until the ball is behind his body.

Watch Mika  push away see how his push away doesn't pull down his body. Fix your balance then push away then post another video.
