Wrist cupped too much. Leaning too far forward at the line. You're throwing the ball with your forearm at the release rather than your arm/shoulder as a whole (which might be a result of your over-cupped wrist at release). You also speed up your steps as you progress the approach.

I would try to address them in this order:

Stop over-cupping the wrist, which may help with:

Releasing the ball with your forearm as opposed to your shoulder and entire arm. Think of this unit as a pendulum.

Then slow your footwork to be consistent which should stop you from:

Leaning forward too far at the foul line. You have so much momentum from those last two steps that your body is still moving towards the pins.

Looks like your finger grips are sticking up too high.  May or may not affect the roll, besides being illegal.

And where exactly is the USBC rule that says your inserts must be flush with the ball?

Section 1. Bowling Ball Specifications

b.   Surface

1.   The surface of a ball must be free of all depressions or grooves of specific pattern, except for:

a)  Holes or indentations used for gripping the ball

b)  Identification letters and numbers,

c)  Incidental chipping or marring caused by wear.

2.   No foreign material may be placed on the outer surface of the ball.

3.   The introduction of metal or any other substance not comparable to the original material used in the manufacture of the ball is prohibited.

4.   If the surface friction of the ball is altered by sanding or polishing, the entire surface of the ball must be sanded or polished in a uniform manner.

While the above rule doesn't specifically state that inserts must be flush, it can be interpeted that way.  The purpose of this rule is to ensure that only the ball surface touches the lane, and inserts that are above the holes would prevent that from happening.

USBC rule book, Chapter 7, Section 1-3:

3.   Plugs and grips may not exceed beyond the surface of the ball.

you need to lay the ball down sooner...(no need to loft it!! unless the heads dry...)
