Brunswick Customatic Fitting Ball

Just got one of these from a pro shop going out of business, the owner didn't have the manuel or instruction guide but has all the inserts. Any one know how to use it or know where a manuel is online?

It doesn't even have the measurments for the letters from thumb hole.

It has half of that sheet looks like got torn off, but I'm unclear as how to use it. I'm assuming the manuel explains everything.
