In thumb what, if anything does left vs right accomplish??

It matches the angle of the hole with the hinge angle of your thumb so you get a clean release and don't blister or build callouses on the sides of your thumb.  --  JohnP

If you bowl enough, you are going to build up callouses. It is from friction and releasing the ball properly will result in callouses. There are correct callouses and callouses built up from blistering.

As far as proper fit, in regards to pitch, you have to start with a proper fit. If your fit is wrong, then pitch is a band aid.
After making sure your span is correct, then pitch is used so that the bowler does not have to grip or squeeze the ball to hold onto, as well as releasing at the proper place in the swing or approach.

You can tell by when you start placing your thumb into the thumb hole, if the thumb is forced into an unnatural angle. As well as any hindurance at the release. The hand should not be forced in an unnatural position to release the ball due to pitch.

JRico -- Agreed, I should have said "abnormal callouses" or "large callouses".  Friction causes the skin to harden, if there were no friction you couldn't keep the ball on your hand.  Something else some people don't know -- bowling is not supposed to hurt your hand.  --  JohnP

the old Coke bottle test taught to us all by Bill Taylor is a very good place to start. This helps to show the hinge angle of your thumb.

when you grab a coke can, bottle or anything else thats shaped as such, the tip of your thumb will point to one of your other fingers or the space in between. depending on where it points, it will tell you a good lateral pitch to start with. I do not have my book handy so I cannot give you the positions that correspond to what pitches. If I recall 0 is when your thumb points towards your index finger. 1/8 right is when it points between your index and middle finger, 1/4 right is at your middle finger and so forth.

These are of course, good starting points and not to be taken as gospel.
