Title: Measuring pitches

When it comes to pitches, where exactly is the measurement taken from?  Does it indicate the distance off center at a certain depth in the ball?  Is it based on moving the jig?  

I'm wanting to learn to punch up my own stuff and I have the equipment, just not too clear on that one thing.
