Help with understanding 90 x 2 1/4 x 45 layout

To the more layout knowledgeable people,

Can anyone describe or post of picture of this layout?  I think I understand the 90 and 2 1/4 parts, but the 45 is throwing me for a loop.

I watched the video a couple of times and even tried to follow his instructions with lines on a piece of paper (failed miserably).  I talked with my pro shop guy today and he was able to show me...MB was to the left of the thumb hole and the pin was to right of the ring finger.  Apparently it would give a very smooth and controlled reaction.  If I can find a ball I wouldn't mind experimenting on I might give it a try.
