What has been your biggest mistake with a customers ball in your proshop career?

Worst mistake for me was early on when I was rushing...

...drilled the span an inch too long!! Don't ask...I was in college and it just wasn't a good day before I got to the shop so I just wasn't paying attention and saw 4" when it was 5".  Felt sooooo stupid when I realized what had happened.  Luckily I had another ball in stock for the customer so he ended up with his ball and I got to try out something different.

I had a pro shop forget to put any glue on a thumb slug.  I took the ball to throw a free game from the shop after it was drilled, and just about broke my wrist.  Half-way thru the backswing of my first throw, the thumb slug slid halfway out of the ball, and I just about threw the ball into the ceiling.
