I ordered one in early January. I picked a stock design, added a few logos and they sent me a proof. I had them adjust some logo placements and they sent me another proof. Once I gave them the OK, it went into production. It took me about 3 weeks to recieve it. I had absolutely no problems with the design, ordering or the product itself. It's definitely worth the investment. Here is a link to my jersey.

It is kind of lacking some helpful info, but what I did was took a stock design I liked, put it in photoshop, and then designed it closely to how I pictured it in my mind. I sent a jpeg of my "design", and they used it as a guide. What I got was actually a better version of the one I created. And I didn't do anything fancy, just cut and pasted logos and tweaked the color of the stock design a little bit. Enough to get the point across of what I wanted. And for me, it worked out really well. Good luck. They are very nice shirts.
