Im trying to put together some sort of lane report sheet.
We are having trouble with our lanes at the moment and genuine problems are not reaching the correct people at our centre. So the managers have asked if we can put together some sort of sheet that people can fill in when the lanes have problems.
Has anyone ever done something similar before or know where I can get one or have ideas to put on it?
It will be something like:
Aproaches:: Sticky, Dirty, Chewing Gum. Comments:
Where you can pick which is the problem then have a comments box for each section. Id like to include Concorce,Aproaches,Heads and MidLane,Backends,Machinary and any others that you may have ideas of.

Are you looking for somebody to make the form, or just to provide suggestions?
I'm assuming you want something that you can print out and have the customers circle/check common issues as well as a space for them to write comments?
Maybe something similar to what some deli/pizza places use to take your order?
You could probably make something in either Word or Excel (or the equivalent program in OpenOffice) and save it as a pdf file.

I've tried to run one off myself. Its in MSWord.
I've put on it Concourse, Approach, Oil Pattern, Lane centre, Lane outsides, Backends, Machinary Faults and other problems. Laid out in a table format.
I've laid them out with a few regular problems like Dirty for both Concourse and Approach etc to circle. Then I've put a comments box at the end for any other problems not covered or to go into detail with the problems that exist.
I cant think of anything else to add so if anyone has other ideas then please do shout them out.
