I'm suprised no one's mentioned that your footwork and armswing are really slow.  Plus you're controlling the downswing.  That will make you lose a ton of momentum and keep you in the "Fluffer stage"(where your speed is below 15mph).  Work on a loose relaxed armswing and take a half step to a step back on the approach and see if you can get your feet to catch up with the rest of you.

It looks to me like you muscle the hell out of the ball to get it up and through the whole swing.
