tdub, when did you have the IT system intalled in the ball that failed ? Also, was the slug glued together by the pro shop or was it purchased from Vise already glued ? In both cases, it could be pro shop error. If the Pro shop glued the IT to the slug, he possibly did not seat it properly and also when he installed the safty molly, he could have hit the Vise tool too hard with a hammer, forcing the IT to raise above the slug causing a weak spot that could break. Also, when the safty molly was installed in the ball, if the pro shop operator, using a hammer, again hit the tool too hard, he could have also damaged the Ball-It, making it brittle which again could make it fail.
Now, I am not saying it has to be the Pro Shop, but it could be. If the operator feels he/her did everyting correctly, have the pro shop call Vise directly and explain what happened. You may also do the same. They want to know of any problems with their system, so it can be corrected, by either them or the Pro Shop.
