How do u get out of a slump?

I have been in a slump for the past two years.I shot my  300s about 2 and a half years ago they were a month apart..I also shot my first 800 series for an 821. I made all the cuts in the tourneys.. It was nothing for me to finish top 5. Now, I Cant seem to find any comfort.. I can barely go plus anymore. I've been battling with my game for a while. There was a point in time where I couldnt do anything wrong.....now i cant do anything right..So my questions are, has anyone else gone through this and how did u get ur game back?

My biggest problem right now that im dropping my shoulder which is causing me to slide to the right rather than straight forward..it throws my balance off...But im just gonna go ahead and grind it out..thanks again for the advice and like i said...ill be back stronger than ever..
